Childhood Vaccinations

All children entering kindergarten and 7th grade are required to have had the following shots (as long they haven't already received these vaccines in infancy). When the child enters school, he or she must present signed proof of vaccination (Department of Health Form 680) from the clinic to school officials.

  1. Ask your doctor, clinic or county health department to fill out the Department of Health Form 680 for you.

  2. Don't forget to take your child's immunization record with you.

Immunizations Required for Child Care and/or Family Day Care
(up-to-date for age):

  • Diphtheria - Tetanus - Pertussis Series
  • Haemophilus influenzae type b (Hib) Series
  • Measles - Mumps - Rubella (MMR) after first birthday
  • Polio Series
  • Varicella
Immunizations Required for Preschool Entry
(age-appropriate doses as are medically indicated):
  • Diphtheria - Tetanus - Pertussis Series
  • Haemophilus influenzae type b (Hib) Series
  • Hepatitis B Series
  • Measles - Mumps - Rubella (MMR)
  • Polio Series
  • Varicella

Children entering kindergarten must have:

  • Haemophilus influenzae type b (Hib) Series

Children entering 7th grade must have:

  • Hepatitis B series
  • Tetanus-Diphtheria Booster
  • 2nd dose of Measles Vaccine (preferably MMR vaccine)

Where to Get Immunized

The Miami-Dade Health department offers FREE vaccines on Saturdays from 10 am -2 pm in various locations in Miami and Hialeah.

Hialeah Immunizations

On June 12, 2003 a mobile van next to the Palmetto Hospital Emergency Room will offer vaccinations between 9 am and 3 pm. Address: 2001 West 68th St.

Mobile Van Program:
